Applications are due by May 1st, 2006

Thank you for your interest in the Our Time Is Now Summer Institutes. Please print all pages and read the application carefully before completing it.

Students who wish to participate must return the following materials by the application deadline of May 1st, 2006.
(Please send all items in one packet, if possible.)

1. a completed application form (Please click on the underlined words to open the pdf file. Then simply print the form, fill it out, and send it back to us with the other required materials.)

2. two letters of recommendation from teachers

3. a completed and thorough statement of interest form

4. a Signature Form. This form documents your interest in the Summer Institutes and should be signed by you, your parent/guardian, and your school principal.

5. an up-to-date transcript (to January 2006)

Put all five parts of your application together in one envelope and mail it postmarked by May 1st, 2006, to:

Applicants will be selected based, in part, on the timely submission of application forms, thorough responses to the essay question, and teachersf recommendations. Due to the number of applications received, you will only be contacted if you are offered an interview.
